I the undersigned, as the parent or guardian of the student named above, grant permission for the person herein named to participate in JAM Omaha 2026, to be held from August 7-11, 2026 at Omaha Chinese Christian Church, located at 4618 S 139th St, Omaha, NE 68137. Furthermore, I, on behalf of myself and any other parent or guardian of the person herein named, do hereby absolve New Heart Music Ministries and Omaha Chinese Christian Church, along with its employees, agents, and volunteers, except where such waiver is prohibited by law, of any responsibility in the event of accident, injury, or loss. In the case of an emergency, I grant permission for my child to receive necessary medical attention, as determined by medical personnel that are present. I consent to take full financial responsibility for any and all medical services rendered. I consent to all JAM policies and procedures as set forth by New Heart Music Ministries, and give my consent for photo/videos of my child to appear in future print, digital, or web publications related to JAM and New Way at New Heart Music Ministries as long as no full identifying information is shown.
